What is primary ovarian insufficiency?

Primary ovarian insufficiency (sometimes called premature ovarian failure) occurs when your ovaries-which store and release eggs-stop working before age 40. You may have no or few eggs. Depending on the cause, primary ovarian insufficiency may develop as early as the teen years, or the problem may have been present from birth.

A woman who has primary ovarian insufficiency is very likely to have irregular or no periods, infertility problems, and menopause-like symptoms. It is difficult, though not impossible, for women who have primary ovarian insufficiency to become pregnant.

What causes primary ovarian insufficiency?

Although the exact cause of primary ovarian insufficiency may be unknown, a genetic factor or a problem with the body's immune systemmay play a role in some women. In an immune system disorder, the body may attack its own tissues-in this case, the ovaries.

Primary ovarian insufficiency may develop after a hysterectomy or other pelvic surgery or from radiation or chemotherapy treatment forcancer. In some of these cases, the condition may be temporary, with the ovaries starting to work again some years later.

What are the symptoms?

The symptoms of primary ovarian insufficiency are similar to those of menopause. Your menstrual periods may become irregular-you have a period one month but not the next-or they may stop. You also may have some or all of the symptoms of menopause, such as hot flashes,night sweats, irritability, vaginal dryness, low sex drive, or trouble sleeping.

How is primary ovarian insufficiency diagnosed?

If your periods become irregular or stop, your doctor will give you aphysical exam and ask you questions about your general health and whether you have other symptoms of primary ovarian insufficiency. Cervical Cancer 

Cervical cancer is cancer that starts in the cervix, the lower part of the uterus (womb) that opens at the top of the vagina.Causes, incidence, and risk factors Worldwide, cervical cancer is the third most common type of cancer in women. It is much less common in the United States because of the routine use of Pap smears.

Cervical cancers start in the cells on the surface of the cervix. There are two types of cells on the cervix's surface: squamous and columnar. Most cervical cancers are from squamous cells.

Cervical cancer usually develops very slowly. It starts as a precancerous condition called dysplasia. This precancerous condition can be detected by a Pap smear and is 100% treatable. It can take years for precancerous changes to turn into cervical cancer. Most women who are diagnosed with cervical cancer today have not had regular Pap smears or they have not followed up on abnormal Pap smear results.

Almost all cervical cancers are caused by HPV (human papilloma virus). HPV is a common virus that is spread through sexual intercourse. There are many different types of HPV. Some strains lead to cervical cancer. (Other strains may cause genital warts, while others do not cause any problems at all.)

Early Cervical Cancer Has No Symptoms

A woman's sexual habits and patterns can increase her risk for cervical cancer. Risky sexual practices include having sex at an early age, having multiple sexual partners, and having multiple partners or partners who participate in high-risk sexual activities.

Risk factors for cervical cancer include:

• Not getting the HPV vaccine
• Poor economic status
• Women whose mothers took the drug DES (diethylstilbestrol) during pregnancy in the early 1960s to prevent miscarriage
• Weakened immune system

Symptoms Most of the time, early cervical cancer has no symptoms.Symptoms that may occur can include:

• Abnormal vaginal bleeding between periods, after intercourse, or after menopause
• Continuous vaginal discharge, which may be pale, watery, pink, brown, bloody, or foul-smelling
• Periods become heavier and last longer than usual

Cervical cancer may spread to the bladder, intestines, lungs, and liver. Patients with cervical cancer do not usually have problems until the cancer is advanced and has spread. Symptoms of advanced cervical cancer may include:

• Back pain
• Bone pain or fractures
• Fatigue
• Leaking of urine or feces from the vagina
• Leg pain
• Loss of appetite
• Pelvic pain
• Single swollen leg
• Weight loss

Signs and tests Precancerous changes of the cervix and cervical cancer cannot be seen with the naked eye. Special tests and tools are needed to spot such conditions.

• Pap smears screen for precancers and cancer, but do not make a final diagnosis.
• If abnormal changes are found, the cervix is usually examined under magnification. This is called colposcopy. Pieces of tissue are surgically removed (biopsied) during this procedure and sent to a laboratory for examination. 
• Cone biopsy may also be done.

If the woman is diagnosed with cervical cancer, the health care provider will order more tests to determine how far the cancer has spread. This is called staging. Tests may include:

• Chest x-ray
• CT scan of the pelvis
• Cystoscopy
• Intravenous pyelogram (IVP)
• MRI of the pelvis

What Is Vaginitis? What Causes Vaginitis?

Vaginitis means inflammation of the vagina. In most cases it is due to a fungal infection. The patient typically has a discharge, itching, burning, and possibly pain. It is frequently linked to an irritation or infection of the vulva. Vaginitis is a very common condition. It is especially common in women with diabetes.

The vagina is the muscular canal from the cervix to the outside of the body. It has an average length of about six to seven inches. The walls of the vagina are lined with mucus membrane.

People frequently refer to the vagina when really they mean the vulva or female genitals generally - strictly speaking the vagina is a specific internal structure. The only part of the vagina that can be normally viewed from the outside (without any instruments or carrying out a pelvic examination) is the vaginal opening.The rest of the area are parts of the vulva, which include the labia majora, mons pubis, labia minora, clitoris, bulb of the vestibule, vestibule of the vagina, etc.   There are several types of vaginitis. The most common are:

• Atrophic vaginitis (or senile vaginitis) - the endothelium, the lining of the vagina, gets thinner when estrogen levels go down during the menopause. This makes the lining more susceptible to irritation and inflammation.
• Bacterial vaginosis - caused by overgrowth of normal bacteria in the vagina. Patients usually have less of the normal vaginal bacteria called lactobacilli.
• Trichomoniasis - sometimes referred to as trich. It is a sexually transmitted single-celled protozoan parasite Trichonomas vaginalis. It may infect other parts of the urogenital tract, including the urethra (where urine comes out of) as well as the vagina.
• Candida albicans - this yeast-like fungal organism is what causes thrush. It exists in small amounts in the gut and is normally kept in check by bacteria.

What are the symptoms of vaginitis? A symptom is something the patient feels and reports, while a sign is something other people, such as the doctor detect. For example, pain may be a symptom while a rash may be a sign.

The hallmark symptoms of vaginitis include itching, burning and a discharge.

• Irritation of the genital area
• Vaginal discharge
• Inflammation - redness, swelling of the labia majora, labia minora and perineal area; mainly because of the presence of extra immune cells.
• Dysuria - pain or discomfort when urinating
• Dyspareunia - painful sexual intercourse
• Foul vaginal odor

What causes vaginitis?

Vulvovaginitis - inflammation of the vagina and vulva - can affect all women of all ages from every socioeconomic and ethnic backgrounds.

Infectious vaginitis makes up 90% of all cases in post-pubescent females. Infectious vaginitis includes candidiasis,bacterial vaginosis and trichmonisasis. Less commonly vaginitis may also be caused by gonnrrhea, Chlamydia, mycoplasma, herpes, campylobacter, some parasites and poor hygiene.

Young girls, before they reach puberty, may also develop vaginitis, but the cause is often different from those for older females. While streptococcus spp causes bacterial vaginosis in pre-pubescent girls, for post-pubescent females it is Gardnerella (both are types of bacteria). Improper hygiene in pre-pubescent girls can transfer bacteria and/or other irritants to the vaginal area from the anal region. Pre-pubescent girls do not usually get yeast infection because their pH balance is different from older women's.

An allergic reaction can cause vaginitis. For example, some women may be allergic to condoms, spermicides, certain soaps and perfumes, douches, topical medications, lubricants, and even semen. Irritation from a tampon can cause vaginitis in some women.
